the water that this nursing home, tastes really nasty. it really needs to be tested.

Recently there have been many senior falls and injuries due to management cutting down nursing staff hands-on care - since the Provena & Resurrection merger (now known as Presence Healthcare).

Just within a few weeks 11-18 seniors had fallen, some with head injuries and fractures, sent to ER, resulting in 2 possible deaths pending IDPH investigation. They had a great reputation, however just within the past two years with new management, the care has gone down drastically, taking a nosedive at record speed.

There is currently an IDPH investigation going on right now due to this.

PLEASE be careful if you admit a dear family member here.

Management will have you believing the falls are due to your loved one's age and debilitating condition, they even have their staff believing this!

It's evident that management cares more about saving corporate $$$$$$$$$$, to selfishly save their jobs and NOT about patient safety.

My family member is a resident there for rehabilitation as a cancer patient. We were told when she was transferred there from the affiliating hospital, she would be evaluated by the on staff doctor within 72 hours. Well it has been a week and she has not even been seen by a doctor on staff there. We have been checked on by the nurses and CNA's and her meds have been changed slightly. When we asked about the doctor visit, we were told by the staff, that this is a normal thing and that the "doctor" is very rarely there and they never know when or if he is coming in. We've also had a few instances with the nurses not being attentive to my family member who was left in a wheel chair for about 45 minutes while they tended to another patient. My family member was not able to get up and get themself back into bed so they stayed sitting in the wheel chair until the nurse came back. There was also an instance where an iv drip was pushed through the port in about 5 minutes when it was supposed to be on a slow drip to take about 15-20 minutes to complete. This caused my family member to be nauseas that evening. The rooms are not usually cleaned nor are the linens changed. The food is also not anything spectacular. It us usually cold when served. The staff in general is usually very pleasant and at some times very helpful. I do not feel comfortable when a staff nurse is unfamiliary what medicine they are giving you and why. Overall after 7 days of my family member being a patient at this facility, I can definitely say i am not pleased.

Not at all, their office smelled fantastic!

We had my mother in this facility for 3 horrible weeks before we pulled her out and moved her to another SNF in the SAME system. We only selected the facility based on good medicare ratings and the fact that the place passed the "sniff" test.
The majority of the nursing staff was poorly trained and left patients sit for an hour when they needed to use the toilet. A night CNA hid a call button and left my mother on an unstable bed side commode that was unsafe. They played game with call buttons by resetting them and leaving. This was traced via computer records with the head nurse.
Overall there seems to be serious administrative issues that are not being addressed. The toilets in certain rooms are too small to take a wheel chair in. The nursing staff is grumpy and pretty bad. The physical therapy staff did not use equipment properly. They did not have mirrors where stroke patients needed them. The head of the OT did not bother reading charts from the hospital/intensive rehab and merely went with symptoms....which were misleading.
The good. The food was decent and the activities people were great. The dining room is great and I think that the floor for long term patients is probably much better run than the floor for short term.
The place we transferred her to has worse food, but the nursing staff has low turnover, they are cheerful and caring. Quite a contrast to McAuley!
