I recently visited Forum Dental and had Kim look at my teeth. He and his assistant gave me an estimate of the cost (treatment plan) involved in restoring my teeth - the cost on the estimate sheet (dated 06/27/19) was $4759. Both Kim and his assistant told me that there wouldn't be any charges for what they did for me the day I was there. I was given an appointment of 07/03/19. When I got home and looked at the cost on the treatment plan, I realized I couldn't commit to that much cost. So I called and cancelled the appointment. After I cancelled, I received a phone call from one of the office personnel stating that Kim said that he would take $500 off of the cost shown on the treatment plan. Yesterday I received a printout from Blue Cross Blue Shield for nearly $100 that Forum Dental had sent to them for what Kim and his assistant did on 06/27/19. Kim and his assistant told me that there would be no charge for what they did - why was charges sent to BCBS? I don't trust Forum Dental and no one else should. (PGJ)

Their primary concern is my safety and comfort, and they always take the time to address every concern of mine immediately. I love how attentive they are here.Their full attention is always on me and it makes me feel special. They always go above and beyond to help me with anything I need. They never complain about anything, and I know I can always count on them. They always make sure to return my phone call as soon as possible. They've never left me waiting. Dr. Cassidy and his assistants are very understanding and willing to help. Being one of the many without dental insurance I have gotten work done off the books, at extreme discounts or they have allowed me to make payments. I will continue to use this doctor until he dies or I die! Such a nice man! I would DEFINITELY recommend him and his staff to any one and every one!

I am so disappointed in the lack of understanding and unwilling attitude displayed by the front desk in helping my elderly mother solve a situation brought on by their scheduling procedures, unknown to new clients. We were told to come in after 11, which was not a specific time, just not before then. The dentist who was helping her, left at noon every Thursday. (I guess we were just suppose to know that) And NO other dentist in the office could look at the 20 year old dentures they sent off to repair a broken tooth, so she could eat normal food again. She has to wait until next week, so the dentist can see her put them in her mouth. We will all live through this, but, I am so disappointed in the lack of human caring and the attitude displayed. When my husband called (unknown to me at that time to plead her case), he was told that the OTHER dentist present was too BUSY to look at her /dentures for the few minutes it would take for her to have her old teeth back in her mouth. Caring, honesty, and decency, I don't think that is too much to expect in these days. I have known one of the dentist there for over twenty years, him and his wife, former scout leaders of our sons back in the Army days, good Christians that would do the world for anyone...God bless them....but, the front desk is the representative that is the face of your service. It was not a very pleasant face that we experienced today.Will I, in the future recommend you? NO. You wouldn't even take three minutes to watch Mama put her old dentures with a repaired tooth back in her mouth, now she will be eating pudding cups and baby food for almost a week. I wish only the best to Dr. Cassidy and his family. They will always be special to me and my son, Clint.

Yes, I received sample toothbrushes and toothpaste

Absolutely! I can always count on them
Did you feel like this provider's office was understaffed?
Not at all, they had plenty of staff members to help me whenever I needed assistance
Does this provider treat every client equally?
Definitely, they make everyone feel equally respected and cared for
