I tried for over a week and half to make an appointment and no one answered the phone. Last time I got the fax machine. If this is any indication on how they run their office, I am glad that I never got through to make an appointment.

Yes, I could tell they were cleaned regularly

Not really, they discouraged any sort of outside advice
Does this provider leave you on hold for a long time when you call?
I usually have to wait on hold for quite a while, but not as long as some other places
Are you confident that this provider will continue working with you until a solution is reached?
No, I'm always worried they're going to settle with a solution that isn't the best

He/she would never do that! Their full attention is always on me.
