Rakiyah T. Jones, DNP, MBA, MPH, FNP-BC is a family nurse practitioner and assistant professor of nursing at Columbia University School of Nursing. He has over ten years of nursing experience including serving on active duty in the United States Army as a Nurse Corps Officer. He received his Master of Science in Family Nurse Practitioner (MS) and Doctor of Nursing Practice (DNP) from Columbia University School of Nursing. He completed his Bachelor of Science in Nursing (BSN) from Rutgers University-Newark formerly University of Medicine and Dentistry of New Jersey. Additionally, he completed a Post-Master’s certificate in Adult Gerontology and Acute Care and a fellowship in Palliative and End-of-Life Care. He is board certified in Family Health by the American Nurses Credentialing Center. Dr. Jones’s clinical interest focuses on complex chronic illness and palliative care in the community with a special focus on Transgender and LGB populations. Dr. Jones previously worked in Manhattan and the Bronx providing primary care in the home and in long-term care facilities. He is providing primary care services for all ages at the Nurse Practitioner Group using an integrative holistic approach focusing on chronic illness prevention, wellness education and quality of life support. His clinical and research interests include complex chronic illness management in the outpatient community setting and Geriatric Transgender health. When not in the office he is busy running a private practice in NJ, serving his country as a Captain in the Army Reserves and spending quality time with his wife and son.
