In our continuing efforts to help patients Stay Healthy™, Eagle Physicians embarked on a rigorous year-long process to achieve the coveted Level 3 “Patient-Centered Medical Home” designation. Level 3 is the highest recognition given by the National Committee for Quality Assurance. This rare honor distinguishes excellence in health care and puts the patient at the center of Eagle’s team-based approach to care and treatment. Being a Certified Patient-Centered Medical Home is one of the highest national honors any medical practice can receive.

Eagle Physicians and Associates, P.A. is a physician-owned and physician-led multi-specialty medical group, governed by our all-physician Board of Directors (pictured below) and championed by many of the other physician owners via involvement in a variety of committees and leadership projects. Because our physicians own the practice, they govern and lead with compassionate and committed awareness that patient-centered, quality-driven care is the number one goal of Eagle. As many other medical groups have elected to affiliate with larger organizations, Eagle has chosen to remain independent and physician-owned as we feel that our physicians themselves are best able to define, support and implement best-in-class quality patient care measures.
