You may be referred for an evaluation if you have a known or suspected condition that can affect your cognitive abilities (such as memory, attention, reasoning abilities). We need a copy of relevant medical records, including any reports of head/brain imaging if you have them. We will have a meeting to discuss your concerns and symptoms, then complete a battery of tests that are designed to measure your cognitive function. Once the tests are completed, we will provide the results to you and your referring physician.
