Cardinal Hospice
Our staff of professional caregivers provide quality care with compassion in the privacy of your own home, within nursing facilities and assisted living. If circumstances do not allow for you to remain in your own home or that of a family member, Cardinal Hospice contracts with several Nursing and Assisted Living facilities throughout our fifty (50) mile service area.Our Hospice team includes two (2) licensed and experienced Nursing Facility Administrators; should you need assistance in this area.

Services
Once a physician determines that a patient has the need and meets the criteria for Hospice Services; ONLY the patient/family can make the decision as to what Hospice provider they want to meet with.
Cardinal Hospice is honored when patients and/or families choose our staff to become their preferred hospice provider. Our patients and their caregivers recieve ongoing comfort and support from our committed, experienced and compassionate interdisciplinary team consisting of physicians, nurses, home health aides, social workers, chaplains, bereavement counselors and volunteers.
When a person is admitted to Cardinal Hospice; an individualized plan of care is developed with the patient, caregiver and our interdisciplinary team: maximizing the quality of life for our patients. Our Registered Nurse Case Managers routinely consults with and updates the patient's Primary Care Physician. Communication with the caregivers are ongoing.
Interdisciplinary meeting are at least every fourteen (14) days to evaluate the effectiveness of the plan of care and update as needed. Special focus is always on the patients comfort level. Additional Services Home Care
