Discharge Assistance
We work directly with physicians, hospitals discharge planners and/or other referral sources to ensure a smooth transition from the hospital at home care.
Patient Assessment
A trained staff member(s) meets with and assesses the needs of the patient with respect to the services and treatment provided. Patients are monitored and assessed to ensure care that is timely and up to date.
Consultations
Staff professionals will meet, as required, with referral personnel or with the patient to solve any matter involving home care treatment or service.
Patient Instruction
A trained staff member of home care professionals ensure that each patient is fully trained on the operation and care of equipment. Instruction is carried out in the convenience and comfort of the patient’s home.
Delivery Service
Equipment delivery, setup, patient instruction and training are provided for FREE by our trained staff.
