Founded in 1976 by the Board of County Commissioners as a model of human caring for the residents of Frederick County, Citizens Care and Rehabilitation Center has met, maintained and exceeded its goals related to resident care and community outreach.

Our mission has grown over the years to include not only long-term care, but short-term rehabilitation as well. We continue to provide excellent care, every time!

The Board of County Commissioners, Citizens/Montevue Board of Trustees, and County Government leaders have supported our efforts by approving new facilities, now being constructed, for both Citizens (skilled nursing and rehabilitation) and Montevue Assisted Living. Construction is on schedule to be completed in early 2012, when we will transfer all of our residents into their new home.

To meet the changing needs of our residents, Citizens offers the following services on a daily basis in our existing building:

Skilled Nursing Care 24-hours a day, every day

Skilled Rehabilitation Care, whether for a few days, a few weeks, or during custodial care

Wound care, including that for diabetic ulcers, pressure wounds, and those wounds resulting from accidents or illnesses.

Nutritious meals and snacks throughout the day, on a rotating schedule. Resident Food Committee meets monthly to choose festive celebratory meals and holiday celebrations.

Activities for all ability levels, including praise & worship, bingo, arts & crafts, movies/music themed events, and even "The Gathering," a special time & place for residents with specific cognitive needs to socialize and have lunch together.

My mom has been a resident now for almost 4 years, and this place has just been going down hill in the past two! Yes, the staff are mostly friendly, but the level of care is rapidly declining. Somehow, mom was able to fall and break both femurs this spring, which almost killed her! They call me for small things, and don't when it's something major sometimes. Mom was recently taken to Baltimore for a follow up appointment, I was never notified that she was even going out of the facility! The last two weekends that I visited, her whit board hadn't been updated for 4, to 5 days.. this tells mom who she has for the day, as well as me! Yesterday, mom's nurse call button was wrapped around the bedrail so tightly, that I couldn't even pull it as far as mom's shoulder, and was on the floor. Mom recently got pinned between the bed, and food tray, causing major bruising. I found that her bed controller wiring was bare, and was touching the metal of the bed!! the staff has to see this on a daily basis, and has done nothing about it. this could have shocked her, or caused the bed to move on its own, causing more injuries! They also don't follow diabetic menu's, they had a root beer float party, and served mom regular soda and ice cream last week, and her sugar level hit over 550! Pleas, if you must put a loved one in this place, make sure you visit frequently, and at different days and times!

Went there to visit a friend and I over heard a nursing assistant verbally abusing a resident. My friend is on the 3rd floor and I noticed that the nursing assistant name was Venice and this isn't the first time that I have seen it being done. I informed the family of this matter
