Dr. Leavens refused to sign CIGNA Insurance Forms for my employer to keep me on FMLA. He was to be the doctor of my choice to treat me for Myasthenia Gravis. But refusal to sign the papers was extremely bad customer service and rather unethical since he was the attending doctor treating me and the disease.
