I left a form that the court required for my grandfathers guardianship. The doctor kept the form for over a week and did not fill it out. He never called me. The receptionist said that we may need an appt even though the doctor saw my grandparents 1 month earlier. I wonder if I should have attached $100 to the form? Very Rude and inconsiderate.
