After repeated phone calls to the Belleville office, I finally got an appointment for 12/14. I spoke with 'Allie" who thought it was Monday, when it was really Tuesday, and she said I filled out the papers on October 28!! Which I did not. She then tried to email the document with instructions but was unable to complete this simple task.. She said she would send it to me US postal service but I have not received it. I cannot get anyone on the phone, only an automated message.
