I was on medical leave from my employer and was referred to a psychiatrist by my primary care physician. Dr. Choksi extended the medical leave. During the third visit, I was experiencing side effects from the meds and he realized that the diagnosis was incorrect and he change the prescription. Although he has a sign in the waiting room stating that he will complete paperwork, the next week, he refused to complete the one-page document for my employer. Since he is the doctor who extended the medical leave, I am now facing losing my job because I don't have the completed form. His assistant stated that he "fills out forms for some patients." I don't recommend this doctor.

I saw Dr. Choksi for my first time and was sort of impressed. He did listen, but seemed to be in a hurry. I wanted to ask him about other possibilities of anti-depressants but he was stuck on the two things he wanted me to take.

I then had to call him a couple weeks later due to having a procedure done where I couldn't start the anti-depressant when he prescribed it for me. I also questioned him about taking one of the two medications he prescribed and did I have to take both of them, as that one really put weight on you. He proceeded to tell me, in a not very nice tone, that he didn't have time to discuss things like this on the phone and to take what he prescribed for me. He was so rude, I couldn't believe it and I spoke very nicely to him and only was on the phone 5 minutes, but that was to much time for him. I won't be going back as I won't be spoken like that by a doctor who acts like he is God.

Absolutely not! I constantly have to repeat and re-explain my situation!
Would you refer this provider to a family or friend?
No, If I had known how terrible they were I never would have gone.
Did you leave the office feeling satisfied with your visit?
No, I left with more problems than I came with!
