Excellent care involves connecting my patients to other skilled providers when appropriate. My intuition is that your needs may better served by XYZ and we’ll work closely together to help you in your journey! I work with patients as long as we’re achieving benefits and refer them on if limitations in my efficacy present). Patient-Centered Care means a whole team of folks supporting that individual and possibly their family too!
