Dear Ms. Harnn,

Today, May 14, 2017, my husband and I visited my aunt, ___, for a Mother's Day brunch. We live in New York State and try to visit as often as possible. I had a few concerns that I am sure you can address for me as I am concerned about the care of my aunt.

1. Does___ have a pair of prescription glasses? During the last several times that I have visited, ___ glasses could not be located. I have given her a pair of my reading glasses. Needless to say, this is a frustrating situation that I am sure you can investigate. I did speak with staff members today regarding the eyeglass issue. Nobody knows. I will gladly pay for her eye exam and glasses if they cannot be located. I believe she had bifocals with metal rims.

2. During my visits, administrators/directors are never seen on the floor to monitor the need of the residents or the aides. Is this a normal procedure? Today, at the brunch, administrators/directors were quite visible at this event for the public. I think you were even taking pictures with your son today. I suggest that supervisors make themselves more visible in a supervisory capacity for all staff under their direction on a consistent basis. To me, public relations events are what they are.

3. How are the events planned for residents according to their interests or abilities?. Yes, I have seen the big calendar that is posted on the wall Too often, I see residents in the dayroom with their heads down or even trying to get out of their wheelchairs. Today, I noticed this again, and called it to the attention of a staff member behind the desk. The television was on, but little else. I am concerned about the mental stimulation for my aunt as well as the other residents.
4. ____ is unable to use the bathroom without assistance. I am sure you have many residents in a similar situation. She was absolutely in a panicked state when a staff member was unable to assist her with the restroom in a timely manner. I personally had to assist her on the toilet. A staff member did come but the wait period was too long and an accident could have easily occurred.
Are their enough staff members to handle these needs? Is this issue discussed at staff meetings?

5. I also noted that a type of "marker" was used to note areas of concern on my aunt's body. This practice seems to lack dignity for the individual. I do understand that the area must be noted, but this identifying procedure seems to be disrespectful. I would like to know if this is standard procedure in all nursing facilities.

I must admit that the outside of your facility is very well cared for and manicured. The first floor, or the entrance floor is very well decorated. My concerns are for the nuts and bolts that are important for the physical, and social, and mental heath of the individuals under your care.

I visit a resident that has been at thiis facility for several years. Overall, I would say that the facility is well maintained. It would be nice if there were additional activities. Since some residents don't regulalry receive visitors, I would suggest that the facility partner with some of the high schools in the area to see if the students would visit with the residents. This would be great community service and the residents would certainly appreciate the 1:1 attention. Some of the staff would benefit from more coaching. I'm not sure if there's an employee recognition program, but this would certainly be a way to promote "engagement" amongst the staff. The facility could then promote the program so that the visitors can see who has been recognized and what wonderful competencies they demonstrated.

Their facility was well organized and clean (Bloomingdale Health Ctr.). I could tell they took a lot of time making sure it was presentable. I can't stop telling people about this place. I couldn't have had a better experience. It was so nice to see them using the latest equipment. It made me have even more confidence in their abilities. The staff is great about turning their phones off when they come into work. Their full attention is always on the patients. The staff was truly one of the best staffs I've ever seen. They genuinely care about everyone who visits.

My only concern is that who will we be dealing with, now that this facility is under new ownership? For example, Ruth Suazo (in Accounting) was terminated (as I learned today form the receptionist). I just sent her an e-mail with an attached invoice from my mother's insurance. Her pension and social security benefits (her only income at this point) is being deposited into Bloomingdale's account) and so Ruth paid this bill on a quarterly basis from those funds. The invoices continued to come to me as her P.O.A. and I would scan and e-mail them to her.
I got no response to the e-mail. I understand that effective today, the new owners have taken over the Bloomingdale facility and its operations. Why have I and others like me (family of the patients in their care) have not been contacted and advised of who we should be contacting for what?
I think most of us would appreciate a letter or e-mail , explaining who the new owners are’ their plans and goals for the future; and receive a list of names, job responsibilities, phone nos. and e-mails of the principals and the staff that we should be contacting (based on the issues of the moment).All I have received thus far is rumour and inuendo (from the staff) during my visits.
So, please clue me in; as well as all the other relatives of patients as to what has happened; what will happen and how are we to begin to relate to your organization on the basic questions that face all of us.

They barely gave me any information at all. I'm going to have to do my own research to get my questions answered. Some staff are friendly, mostly aren't, I didn't feel welcome there. They didn't help me as much as I was hoping and didn't answer all my questions. Director of Nursing was rude, i didn't like her attitude when we met her. I heard from a co-worker whose Aunt is on the same Nursing Home that she only cares if the patient is a Private Payor, if is on Medicare or Medicaid then you will not get the right treatment. I care less about this place because I already moved my mom to a nearby nursing home. But do not be surprised if you are given a tour by this Director of Nursing (Suzette Cornelio)
