My disabled mother has been a resident of Southlake Nursing & Rehab for 5 years and we would not have her live anywhere else in Jacksonville Florida. The facility is spic and span clean and deep cleaning is done often. Custodial is amazing and cleans each room daily, including making up the bed and emptying the trash. Overall, the staff is very friendly, like a family and solid relationships of trust have been built over the years. Most are very protective of their assigned residents. She is bathed on a regular schedule at least 3 times/week. I am continually informed of any concerns, medical treatment, etc.by the administrators and nursing staff and there is an abundance of activities that keep her very busy. The high-tech computer system has a resident profile and leaves no room for error in administering required medication. The gourmet cooks provide an abundance of healthy and flavorful meal three times a day, and comply with safety, sanitary rules and even custom diets for each resident accordingly. They even provide periodic snacks door to door. The laundry crew alleviates my burden of doing extra laundry and there is a hair salon, PT room with state-of-art workout equiopment and awesome therapists on site. There are a variety of daily/weekly Church services, Entertainment Bingo and Educational activities and Resident/Family meetings on a regular basis. I have seen negligence in other nursing facilities and can honestly say we are blessed to have found such a great home for my mother. I live 5 minutes away and you can believe I pop over unexpectedly at various days and times to ensure my mother is being well taken care of. I have been extremely pleased with the consistency of all three shifts. There is a ZERO TOLERANCE for anyone that dare physically or verbally abuses any resident or family member and the administrators don't hesitate to take action to terminate anyone not following the regulations and safety procedures. Southlake allows my family to have peace of mind and sleep at night. THANK YOU SOUTHLAKE!

This is the worst nursing home I have ever been to. When you first walk down the halls, the smell of urine and other bodily fluids overwhelm your senses. Once in awhile an aid or nurse will go down the halls spraying air freshener (like that will help the odors)! The residents are usually trapped in their wheelchairs and placed in the hallways and they are just left there. Sometimes they will be crying, shaking, and screaming for help. Some of the aids never smile and seem extremely irritated to be at work. They do not seem to want to help anyone. Those aids can be rude and ask "what do you want" after you call for help. The worst is that it takes a long time for a resident to get changed. They sit in their wet diapers (probably for hours) because my resident is always having problems associated with that. During meal times, they seem extremely short staffed and will not help the resident with anything! The list can go on and on. I would not place a loved one into this nursing home! Consider your other choices!

The most disgusting nursing home that I've ever entered. My father-in-law was placed in this facility last week by his wife (who has control of his medical decisions). We visited for the 1st time today. The premises smelled of urine and feces. The residents were sitting in the hallway in their wheelchairs. We could barely maneuver the hallway. My father-in-law had been alert and aware last week while in Memorial Hospital but was almost comatose today. The nursing director indicated that he was being dosed with Atarax because of his unruly outbursts. We could not arouse him. We questioned the nurse and assistant and they had no clue as to the care being given him. I would not put my dog into this facility.
