He was the best psychiatrist I ever had.

It's been 27 years. I am trying to get updated medical history for myself for Social Security Disability. They require I go way back to my time of depression in 1991, and my hospital stay there. Dr. David Shwartz was my Doctor at the Carlyle center. I was not in the addiction ward. I was in the Psychiatric ward. He never had me going to NA or AA meetings and Such. I was on watch for my first couple of days for suicide. I could really use some information into why he didn't think I was a drug addict , and put me on Lithium, that gave me my life back. I would appreciate any help you can forward to me. Thanks, and have a great weekend.
