They DO NOT provide insurance approved statement of services even when asked. They DO NOT submit to insurance companies they are not already associated with. Expect to pay IN FULL IN ADVANCE. Even if services are covered by your insurance company you will never know because Fontana refuses to provide an insurance approved statement. They also IGNORE letters from your insurance company.
I cannot comment on services provided because they were for my son. I just paid the bills.
