Housecall Providers is a nonprofit medical practice delivering in-home physician, nurse practitioner and physician assistant care to medically fragile, homebound older adults and persons with disabilities. We provide quality, proactive primary medical and hospice services. Because of our services, our patients have access to medical care that often gives them the opportunity to remain in a home environment and to maintain their dignity, in familiar surrounding where they are comfortable.

At Housecall Providers, our goal is to integrate primary and palliative care through the end of life. We have an entire team of hospice specialists to meet the full range of medical, social and spiritual needs of our patients.

Housecall Providers Hospice, in concert with primary care, serves our community by offering physical, emotional and spiritual support to persons completing life's journey and those who love them. We strive to add quality of life to this sacred time.

We just used Housecall Providers Hospice for our dying mother who had Alzheimers and was living in an assisted living facility. The hospice team came together quickly and were supportive and there for us every step of the way. Mom went quickly, so it was difficult for us to accept all of the information and emotions at once. The hospice nurse spent a lot of time with us and our Mom and was very sensitive to her needs. We feel she got the best of care in her dying days. She arranged for a chaplain and worked well with the care giving staff at her facility. She even followed up with us days after Mom passed, and made a long distance call to talk with our sister on the East Coast to offer support and answer questions. The woman Mom shared a room with was in Hospice at the same time with a different provider. There was a stark contrast in the care and concern. I would recommend Housecall Providers Hospice to anyone who needs that service.
