in early 2011 I had an allergic reaction to a drug (penicillin) that my files listed as a serious problem (Augmentin) My chart was clearing flagged as I saw whenever I went to her. She didn't check the chart and prescribed from her home a drug with this in it and I ended up in the ER and hospital on IVs for 4 days and with a multitude of health issues which the specialist told me were a direct result of my immune system being brutally attacked.
