Dedicated to changing health care for the better, one patient at a time.

By working with hospital case managers and community providers, Mid-Delta has been able to cut hospitalizations by one-third (33%) over a year's period of time for exacerbation of chronic medical conditions, such as Congestive Heart Failure (CHF), Diabetes and Hypertension-related illnesses.When it comes to delivering safe, cost-effective, high-quality healthcare, nowhere is there a more applicable environment than the patient's home.
