FYI – Please be aware this ENT (Dr. Kampsen) when filling out initial patient / profile information requires you to sign "disclaimers for care" and closed "attorney mediation" forms. In plain language, before he agrees to see you, you must sign away your rights (or limit those rights) to sue him if his care does you physical harm. Please consider this doctor-patient requirement before being caught off guard in the office like I was.
