I thought this would be a good place for rehabilitation following my back surgery, was I wrong. I wouldn't put my dog in this place. I called for pain medication and a nurse came in, whom I had never seen before. She dropped my pain medicine on the floor, picked it up, put it in cup with other medication, stirred it around with her finger. This was in full view of my visitor and myself. When I told her I didn't want to take medicine from the floor, she told me if I didn't take it I would have to wait four more hours. I later found out this was not true, she could have filed an incident report and gotten more pain medicine immediately. This so called nurse probably felt it was beneath her to be giving out medication because she was the Assistant Director of Nursing and the nurse, whose shift it was was out sick. Twice an employee went thru my personal belongings and stole money. The second time the Administrator was notified and when she came in, it was quite obvious she didn't believe me. When I said this to her she just stood there, never saying a word. There were a few aides and some nurses that were very kind to me. You could push the call button and sometimes it would be answered, but help could take an hour to come, but more often it wasn't answered. The administrator, director of nursing and assistant director of nursing were very, very rude, never addressing me in a decent tone of voice. Think long and hard, then think again before putting a loved one or yourself in this facility. If you want to be treated rudely that his is the place to go.

I was transferred there from the hospital. The first night I hit call button for assistance to go to the bathroom. I kept hitting it, then started yelling out after an hour and finally wet the bed. After two hours someone finally came in. She said call button wasn't working. After that experience I was treated good by the immediate staff but the administrative staff were not friendly and were extremely rude. I could not wait to get out of there. The physical therapist was nice until one morning she came to my room and saw one of the heating pads they use on my bed. I was having severe sciatica attacks so one of the employees brought it in from the physical therapy room. I told her I didn't remove it. After that she never spoke to me again. Why, I have no clue! Staying there was the worst experience and waste of time and money,

09/08/2015 An Interested Visitor Writes: Except for the rehab department, this facility is horrible. The management is cold, uncaring, and has an excuse for everything that goes wrong, which happens daily. Nurse and aide turnover seems to be weekly. Push the nurse's call button and you will be lucky if anyone comes to help within an hour. If you do not have family there constantly, your loved one sits in dirty diapers, does not get their food cut up, no fresh water, and no daily assesments of their physical mental condition. These people simply do not care about their patients. Mgmt walks around with their noses in the air and never speak. Also, very, very few activities for the patients. Very limited menu and food is average at best. My friend had to move her mother after 3 days, my co worker had to move her husband after a week, and after six long weeks, we finally moved my dad. So dissappointed! I cannot even believe that anyone would subject our seniors to such negligent care. So sad.

To begin, this so called "skilled nursing facility " does NOT even deserve a one star rating. Unfortunately, my father was required to have inpatient rehabilitation and was transferred here from the hospital. The experience was more than disappointing to say the least. Having never been here, I was overwhelmed by the smell of urine as I walked down the hall. After the first night I could understand why. For patients in need of help getting to the bathroom, the wait can be anywhere from an hour to three( as my dad experienced in several occasions). This leaves the patients with no choice but to have an accident in the bed. When I spoke to the administrator about this, she responded with " no big deal, we will just clean it up, that's what we do". How about staffing your facility to eliminate patient wait times?! In addition, in two occasions they forgot to serve my dad his meal. Again, addressed the staff only to receive a confused look and a shrug. Multiple times we had to address the charge nurse concerning the fact that they were not checking his blood sugars ( he is diabetic), he required pain medicine every 4 hours which would take just about that long to receive the first dose he had requested. When addressing the charge nurse about anything her response was consistently rude. She even discussed her negative feelings toward other staff members. Very Unprofessional!!!! As we sat in the "common area" with other patients and their family members, we all communicated the fact that we felt the need to stay all day from morning to bedtime with our family member that is a patient there because we were too concerned they would not be cared for. I, myself, was the one that changed my fathers bed sheets( they only require them changed once a week) , retrieved water, food , blankets and towels. At one point I had to clean the bathroom because it hadn't been done in a sanitary manner. After several unsuccessful talks with the administration, I contacted the Vice President of the facility. I left several messages advising I had concerns and have still not received a return call. Shocked!!! This place is less than appealing and more on the line of scary based in the LACK of care, compassion and respect they display. Aside from the rest of the staff, there are two Staff members I would like to name and give them a big "hats off" and "thank you " to Erin and Ebony! Those girls were the only two that made me feel okay when leaving my dad because I knew he was in good hands with them. Unfortunately the only worked a couple shifts in his two week stay. To the charge nurses, the so called administrator, and the nursing director .... Shame on you!! You're rudeness and lack of compassion and respect certainly don't make you the right person for your jobs. Glad to have moved my dad out of there and never plan to return. For those of you considering this facility , beware of the smoke and mirrors they try to throw your way... It's all fake. Please see this place for what it really is!!

My husband was sent their for rehab due to a fall. It was worse than I had ever expected. The "Administrator" is there to act as a sounding board for everyone to go to voice their concerns, nothing more. She is totally incompetent to run a facility. The "Director of Nursing" listens to you and concerns concerning the behavior of her nurses, when it is a disgrace to the facility, will speak to the involved nurse. Speaking of nurses, during our stay, I only met 2 that had nursing skills and compassion for the patients there. The facility is dirty, smells of urine continuously, and did not have a bed large enough to accommodate my husbands 6foot 1inch frame. The linens looked like they were "bar rags", and sheets were changed every 2 days, unless you went to the storage area and got fresh sheets and changed them yourself. Now for Doctors...what a joke...my husband was there approx. 1 week before he saw The Doctor...which knew nothing about his situation, as a matter of fact, I dont think anyone ever looks at a chart to see why a patient is there. The nurses were rude, uncaring, no help at all and when you have to wait anywhere from 1 to 3 hrs to get your prescribe medication, that is not a "skilled nursing facility", It does not speak well of your facility when families feel they need to hire outside 24 hr. nursing care, or try to work the family members in shifts because you feel your loved one is not going to be taken care of when someone is not there. This facility does not deserve the 1 star rating, but the lowest choice I had. Please think twice before putting anyone you care about into this place, you will be so very sorry. I know this facility has a lot of medicare patients, but they are patients and should be treated with respect and dignity, as well as the family and friends that visit.

Over all it was a horrible experience. The woman (administrator) in charge is extremely arrogant to family members and very rude to the people who work there. I asked her a question once and all she said was "Im on the phone.." Then as i waited outside her office, she yelled at the house keeping employee because she was "behind", she didn't just yell at her, she made her be on the spot in front of a lot of patient family members. I agree with the post before me, very unprofessional.

Their equipment really needs to be updated right away. It looked ancient. Their staff was somewhat friendly, but they didn't go out of their way to make me feel welcome. There is no way I ever would have gone to this place if I had known how horrible it was going to be. I'm definitely not going to be recommending it to any of my friends or family. Their building was easy enough to find. The staff is great about turning their phones off when they come into work. Their full attention is always on the patients. The administration are completely rude and don't listen to anything. I actually heard the adminstrator yelling at a CNA out in the hallway for everyone to hear. Extremely unprofessional. Will not be taking my mother back.

I would recommend this facility because their nursing staff are quite skilled and super friendly. However, their Chief Nurse appears quite young for such an important position. While the meals are good for patients, some dinners are poor. The cook is very friendly. Because of the well skilled nurses (LPNs and RNs) I would recommend the place. However, the facility's doctor is only there one day per week, generally. This is typical of some nursing homes, because few doctors want to invest the time at a nursing home, given it's not profitable for those doctors. While the nurses are in contact with that doctor throughout the week, I would recommend such a facility only to patients without serious health problems requiring more visualization by physicians. There are places in the building for patients to walk. However there's only a tiny yard. While the place is quaint and homey, the semi-private rooms are very small, with minimal room for visitors to be comfortable. The bed hardware is old. Private rooms appear a good size. There are roughly 12 parking spaces (includes visitors). Fortunately the Chinese restaurant next door allows visitors to park all day. The parking lot was lost due to construction of larger nursing home.
