My Grandma had a stroke in 1993 or 1994 and was in the nursing home for about a year and a half until she passed away there in January 1995. We were happy with the services they provided her.

My mother was there, unfortunately, for rehab after a hospitalization. I knew immediately that it was not going to be a professional and caring facility by the manner in which we were admitted. There was no information given concerning a schedule for meals, rehab sessions, patient comfort, etc....and when I went to find such things out, I felt like I was being an annoyance to the staff. We also had personal items stolen there, by what I believe to be, the staff. Just a bad, bad experience. Look elsewhere for care.

Wish I could leave -10 stars. This place is deplorable. It stinks of poop and urine. The staff is horrible. I took several concerns to the DON, Shannon, and she could not have cared less. She called me a liar. I had concerns for my Mother and her care and was told by Shannon that there was no way that her staff would be abusive to any residents. PLEASE CONSIDER A BETTER PLACE FOR YOUR LOVED ONE!!! I called State and they are investigating. I hope that they shut that hell hole down

My mother is here currently as of 1/18/14. She was put to bed without a depends on and not checked all night. She's 90 years old and has ALS, she's supposed to be here for rehab. She had urinated and had a bowel movement at some time during the night and was laying in it when we arrived at 7 AM. Her private area and behind was extremely red nearing severe rash stage. How un-dignifying to treat someone this way. We asked for the head nurse to come to explain how this could happen at 11 AM she still hadn't showed up and she was there. She refused to come and talk to us. I WOULD LOOK ELSEWHERE FOR CARE and we will be filing complaints with every state and federal agency we can find who governs this place.

My aunt was recently a short-term resident here. She is mentally challenged and they were so nice to her, very patient and kind. The place was clean, decorated for Xmas and I was greeted by staff each time I passed them. They knew her case, helped with getting the correct meds and moved her to a room close to the nurses' station so they could keep an eye on her. When it was time to discharge her they caseworker jumped to help get home health set up and even took on Medicare for us to secure it.
A very nice facility and I would put my family there again.

If the staff seem not to have answers it's because they're new; a typical staff member lasts about two weeks. Why? Because the pay is crap; $10 an hour for an experienced CNA, and they don't pay overtime, they don'y pay holiday pay, there is no vacation time, and many of the staff complain that they struggle with administration to get back they pay they are constantly being being shorted out of on their paychecks. Many staff quit their positions. There is a daily high incidence of call-in and/or no-shows. This leads to terrible staffing; sometimes two to four staff to cover 40 patients. The state is constantly investigating complaints and it's a miracle they continue to stay open (makes you wonder if there's not some payola going on). If there were negative stars this place would get them. Don't send your loved one there to get ignored, hurt, or allowed to die.

Liberty Terrace has alot of great staff that truly care for their residents. Alot of residents that come for rehab to home have very positive things to say about their experience. The food is very good! New management with an attitude of high expectations.Things are looking up!

I feel that Liberty Terrace has been hiring new staff in last 6 months. Most of the previous staff have left and went to work at another facility .I would be concerned for the other facility that has hired this rude,lazy staff. That did not take care of their patients. If you had issues with Liberty Terrace you should rethink this now that they got the lazy negative staff out and are putting good staff in place.
