Jefferson House, a department of Hartford Hospital since 1873, offers a continuum of care on its campus to meet the needs of the aged and infirm. Accommodations and services include an Independent and assisted living complex, sub acute, palliative and long term skilled nursing beds, a rest home with nursing supervision and an adult day healthy center. A team of highly skilled professionals who are experienced providing compassionate care staffs our facility. The therapeutic team includes an on site medical director, a physiatrist, professional nurses, certified nursing assistants, occupational, physical and speech therapists, recreational therapists, social workers, registered dieticians and chaplain. Together this team accesses every new patient and creates a personal care plan specifically suited to the individual’s needs.
