Our rehabilitation team at OakCreek Nursing and Rehabilitation Center provides physical, occupational and speech therapies for both their skilled nursing and long-term care residents. Their skin management program and therapy services are directed by a written plan of care structured specifically for the needs of each individual resident. OakCreek Nursing and Rehabilitation Center (830)875-5606 has a wide range of healthcare professionals who are dedicated to designing your loved ones plan of care. Their staff consists of RNs, LVNs, Certified Nursing Assistants and Restorative Nursing personnel who are available 24 hours a day. They accept admissions 7 days a week. The admission process can be started by a physician, social worker, case manager, discharge planner or by the prospective resident and their family. They understand the admission process can be a very stressful time for residents and their families so they have made their admission process as easy as possible for families and their loved ones. Their staff takes great pride in their jobs, and their caregivers are there to make sure our residents and families are as happy and comfortable as possible. At OakCreek Nursing and Rehabilitation Center they are dedicated to providing quality care in a home-like environment. Their facility has been designed to feel less like an institution and more like a home. They encourage their residents and their families to add any personal touches to make their living area as home-like as possible. Mealtime is given high priority with delicious and nutritious meals served three times a day in their main dining room. They offer daily activities that are designed for their residents social well-being and individual needs. Some of the activities they offer are exercise, music, games, religious activities, community outings and social group activities. They also offer in-room activities for their residents who are unable to attend activities in a large group setting.
