0 star should be an option. This provider INCORRECTLY FILED INSURANCE form. Misspelled doctor name, IEN number incorrect, doctors license number was incorrect. Delta Dental coverage was available &Delta dental went above and beyond trying to get provider to submit correct information several times. Office staff, Daisy was so rude, disparaged me loudly in front of other clientele proclaiming that I didn’t pay my child’s bill. To add insult they turned me into collections. Bonneville collections is nothing short of mean and rude themselves. My credit has always been outstanding until now. Im so humiliated that I was sent to collections, all because of this office incompetence.
